In the event of (1) an accident involving the hospitalization of 3 or more employees within 30 days of a work related incident or (2) the death of an employee, employers must report to OSHA within 8 hours. Employers may call the national office at 1-800-321-6742 or provide a report in person at a local OSHA office.
